What Happens When You Stop Taking a GLP-1?
The medication's effects fade over weeks as it clears, appetite signaling returns toward baseline, and the randomized trial evidence shows substantial average regain within a year of stopping. What does the strongest evidence show about stopping?
Two randomized withdrawal experiments anchor the topic. In SURMOUNT-4, everyone took tirzepatide for 36 weeks, then half were switched to placebo without knowing it. Over the next year the placebo group regained, on average, about half of what they had lost, while the continuing group lost further. The STEP 1 extension told the same story for semaglutide: about two-thirds of lost weight regained on average within a year of stopping, with cardiometabolic improvements receding alongside.
Those are population averages under trial conditions, where stopping is abrupt, blinded, and unsupported. They establish the baseline expectation: the medication is doing ongoing work, and removing it abruptly usually shows you that. Individual results vary and are not typical.
Then why do some newer studies show much less regain?
Because real life is not a withdrawal protocol. Real-world analyses that followed patients after discontinuation have reported strikingly smaller average regain in some cohorts, and the differences are explainable: real patients often taper rather than stop cold, restart when regain begins, stop after reaching maintenance rather than mid-loss, and keep behavior changes the trial design deliberately isolated away. Selection matters too, since people who stop because treatment worked differ from people randomized to placebo.
Read the two literatures as answering different questions. The trials answer what the drug itself was contributing. The real-world data answers what happens to actual patients navigating cost, supply, and preference with a physician. Neither says regain is inevitable; neither says stopping is consequence-free.
Why does hunger come back so loudly?
GLP-1 medications quiet appetite signaling while present. As the drug clears over roughly a month, that signaling resumes, and it resumes into a body that has been defending its previous weight all along: ghrelin runs higher after weight loss, satiety hormones run lower, and energy expenditure drops more than the smaller body alone predicts. Many patients also describe the return of food noise, the background preoccupation with eating that the medication had switched off.
Naming this as physiology matters, because the alternative story, that regain is a personal failure, is both wrong and corrosive. The rebound is the documented biology of weight regulation. Planning for it beats being ambushed by it.
